Health Insurance Portability and Accountability Act of 1996
The purpose of the Health Insurance Portability and Accountability Act of 1996 is to ensure confidentiality of “individually identifiable information.” It governs all individually identifiable information in any form including written, oral or electronic.
Hudspeth Regional Center is permitted to use or disclose protected health information (PHI) to the individual and to carry out treatment, payment, or health care operations: and pursuant to and in compliance with an authorization.
Identifying information includes name, geographical subdivisions smaller that a state, address, relatives, employers, Dates of birth, admission, discharge or death, phone number, fax number, e-mail address, URL, SSN, medical record number, health plan number, account number, device identifiers, certificate/license number, vehicle or other device serial number, Internet protocol address number, finger or voice print, photographic image or any other unique identifying number, characteristic or code.
Employment records maintained by Hudspeth Regional Center in it’s capacity as an employer are excluded from the definition of protected health information.
