Support Coordination Process

During the evaluation process, the Support Coordinator will meet with the person and family to discuss the process and services that are provided through the waiver. Based on the needs and desires of the person and family, as well as the recommendation of the team who evaluated the person, the Support Coordinator will discuss the services that are available. The family will be offered a choice of providers for each service based on availability in their area. An Individualized Plan of Services and Supports is developed and, along with other required documents, submitted to the MS Department of Mental Health’s Division of Community Services Office for review. Once the documents are found to be in order by the Community Services Office, the information is forwarded to the Division of Medicaid for approval/disapproval. If approved, a Support Coordinator is assigned.

Support Coordination is responsible for linking people to resources in the community in order for their needs to be met.  The Support Coordinator offers a choice of services and providers and conducts continuous follow up to ensure the person is happy with their services and supports.  The Support Coordinator coordinates, facilitates, writes, and revises the Plan of Services and Supports.  The Support Coordinator speaks with the person/legal representative/family monthly and sees them quarterly.  Support Coordination monitors the implementation of the Plan of Services and Supports and all Activity Support Plans.  Support Coordination is responsible for checking the eligibility for services and supports and assisting with annual recertification.
